This form is used to request or replace an ID card.  There is a $3.00 fee associated with replacement.  You must check your BOSS Gmail account for a response to your request.  If you do not receive a response within a few days, please email Ms. Rucker (rucker@nycboss.org) for a status update.

This form is used when requesting use of a BOSS locker.  Only BOSS locks can be used to utilize lockers.  Once complete, please return the form to Ms. Leon in the main office, 543C.  She will follow up with you with further instructions.  

Warning: Locker usage is a privilege and should only be used for outerwear and gym clothes.  Hall passes may not be used to retrieve materials needed for class.  Use of the hall pass to retrieve items in your locker will result in temporary suspension of locker use. 

This form is used to request or replace a MetroCard.  You MUST see Ms. Leon in the main office, 543, at the end of the day for further instructions.  Replacement may take up to a week or more depending on availability.  If you would like a status update, you should email Ms. Leon (leon@nycboss.org).

Please fill out this form to request a tour of the Business of Sports School. A response will be e-mailed to you upon 48 hours of completion. Thank you for your interest!

  • pens (black/blue) must bring at least 2 each day (buy at least 20 at the beginning of each marking period (6 times a year))      

  • three 1-2" 3-ring binder with 5 dividers each    

  • three spiral or bound notebooks        

  • graphing calculator, (TI-83) recommended      

  • loose leaf paper (buy at least one ream per marking period)      

  • one ream of 3-hole graph paper      

  • weekly 2019-2020 academic planner, suggested Staples: 5½” x 8½” (we will provide one if you don’t have one)     

  • pencils (at least 20 per marking period)      

  • one manual plastic pencil sharpener      

  • highlighters (at least 2 per marking period)

  • pen/pencil case      

  • one combination lock      

  • clear protractor and compass      

  • clear ruler      

  • one 3-hole puncher to keep at home      

  • five 2-pocket folders      

  • five packs of Post-its
